PREPARING FOR YOUR VISIT

We want your visit to feel seamless from the moment you arrive.  A few simple preparations can help us make the most of our time together and ensure your appointment runs smoothly.  

Before Your Appointment

New patients will be asked to complete registration and required forms through our secure patient portal before their visit. 

Please bring:

  • A current photo ID
  • Your insurance card, if we will be billing insurance
  • An up-to-date list of medications
  • Relevant pathology reports, laboratory results, or outside medical records that may help with your care
  • The name and contact information of your preferred pharmacy

Please notify us before your appointment if your insurance coverage or contact information has changed.

Arriving + Parking

Capital Skin & Laser is located at 500 Taylor St, Suite 400, at the corner of Huger Street and Taylor Street in downtown Columbia.

Complimentary on-site parking is available in the building lot.  Several spaces are reserved specifically for Capital Skin & Laser patients, but you are welcome to park in any available space in the lot.

Because the surrounding streets are one-way, we recommend allowing a few extra minutes on your first visit.  The parking lot may be accessed from Taylor Street and Hampton Street, which travel in opposite directions. 

For convenient passenger drop-off and pick-up, a roundabout at the building entrance allows direct access to the front door.

Once inside, take the elevator to the fourth floor.  Capital Skin & Laser is located in Suite 400. 

Arriving For Your Appointment

Please arrive with enough time to complete check-in before your scheduled appointment.  Arriving late may limit what we are able to address during your visit, or, when necessary, require rescheduling so that we can provide appropriate care without delaying patients who follow you.

Some laser and cosmetic appointments require additional preparation before the procedure.  If your appointment requires early arrival, topical numbing, or other preparation, our team will provide specific instructions in advance.

Full Body Skin Exams

A full body skin exam is dedicated to evaluating your skin for suspicious lesions and signs of skin cancer.   

To allow for a thorough examination, please come prepared to change into a gown.  We recommend arriving without makeup, heavy hairspray, or nail polish, as these can obscure areas of the skin, scalp, and nails that need to be examined. 

Other concerns—including acne, hair loss, rashes, or management of an ongoing dermatologic condition—require a separate appointment.  This allows us to give each concern the time and attention it deserves.

Cosmetic Consultations

An initial cosmetic consultation is required before receiving cosmetic services at Capital Skin & Laser.

Your consultation is designed to evaluate your skin, discuss your goals, and develop an individualized treatment plan. When appropriate, advanced skin imaging may be incorporated into the consultation.

The cosmetic consultation fee is $150, due at the time of booking. $100 may be applied toward eligible cosmetic treatment performed within 60 days of the consultation.

A Note About Appointment Time

Your appointment time is reserved specifically for you. Arriving late may limit what can be addressed during your visit and, in some circumstances, may require rescheduling (see Office & Financial Policies).

INSURANCE & BILLING

We believe financial expectations should be clear from the beginning.  Our team is happy to help you navigate the billing process and understand what to expect from our practice.

Using Your Insurance

Capital Skin & Laser will submit claims to participating insurance plans for covered medical services.  Please bring your current insurance card to each visit and let us know if your coverage has changed. 

Insurance coverage is a contract between you and your insurance company. Benefits, deductibles, copayments, coinsurance, coverage determinations, prior-authorization requirements, and medical-necessity decisions are established by your individual plan.  Verification of insurance benefits by our office does not guarantee payment by your insurer.

You are financially responsible for any portion of your care not paid by your insurance plan, including applicable deductibles, copays, coinsurance, non-covered services, and services determined by your insurance company to be not medically necessary. 

Copays, coinsurance, and un-met deductibes are due in full at the time of service.

Medical vs. Cosmetic Care

Medical services are billed according to the care provided and, when applicable, submitted to insurance. 

Cosmetic services are elective and are not billed to insurance.  Payment in full is due at the time of service.  We accept cash, credit cards (Visa, MasterCard, American Express, Discover), debit cards, and Apple Pay.  Personal checks are not accepted. 

Cosmetic Consultation

An initial cosmetic consultation is required before receiving cosmetic services. The consultation fee is $150, and $100 may be applied toward eligible cosmetic treatment performed within 60 days.

Treatment Packages + Products

Treatment packages must be used within one year from the date of purchase unless otherwise stated in writing.

Product sales are final and nonreturnable except when required by law or specifically approved by the practice.
