Your appointment time is reserved specifically for you.
Please provide at least 24 hours’ notice when cancelling or rescheduling a medical appointment, and at least 48 hours’ notice for Mohs surgery, surgical procedures, laser treatments, and aesthetic appointments. For Monday appointments, notice must be received by the preceding Friday at noon.
Late cancellations and missed appointments will incur the following service fees that are automatically charged to the credit card on file.
Certain cosmetic and laser procedures require a non-refundable deposit of 20% of the procedure cost. Cancellation in less than 48 hours and will result in forfeiture of this deposit, and a new 20% advanced deposit will be required to reschedule.
Missed appointment fees reflect reserved physician and staff time, specialized equipment, and appointment availability that cannot be reallocated to serve another patient on short notice. These fees are the patient’s responsibility and are not billable to insurance.
Repeated missed appointments or late cancellations may result in dismissal from the practice.
We value your time and are committed to seeing patients as close to their scheduled appointment times as possible. We will make every reasonable effort to accommodate late arrivals when doing so will not delay patients who have arrived on time or compromise the time and attention needed for your care. Depending on how late you arrive and the length and nature of your appointment, we may need to see you later in the day or reschedule your appointment. If your appointment must be rescheduled due to late arrival, the applicable missed appointment fee will apply.
Copays and other amounts due at the time of your visit are collected in full at the time of service. Patients are financially responsible for any portion of their care not covered or paid by insurance including deductibles, coinsurance, and non-covered services. We accept cash, credit cards (Visa, MasterCard, American Express, Discover), debit cards, and Apply Pay. Personal Checks are not accepted. We do not participate in CareCredit or other healthcare financing programs.
An active credit card on file (CCOF) is required for all patients. The credit card must be in the name of the patient or the patient’s authorized representative. Card information is stored securely through Capital Skin & Laser’s payment processor. Credit cards on file are only used to medical and/or cosmetic dermatology services on the day of service, deposits for services agreed to be rendered at a future date, cancellation & no-show fees, and any additional purchases or services.
Cosmetic services are not billed to insurance and are due in full at the time of service.
An initial cosmetic consultation is required before receiving cosmetic services.
The consultation fee is $150. $100 may be applied as a credit toward eligible cosmetic treatment performed within 60 days of the consultation.
Package treatments must be paid in full at the time of purchase and used within one year from the date of purchase. Packages are non-refundable, non-transferable, and expire after one year.
Please submit prescription refills through the patient portal and allow up to two business days for processing. To receive prescription refills, you must have been evaluated by Capital Skin & Laser within the previous 12 months. Some medications require more frequent follow-up or laboratory monitoring. Refills may be declined or limited when prescribing without reevaluation would not be clinically appropriate, or at the discretion of the clinician.
To protect patient privacy and allow our clinicians and staff to provide care without distraction, unauthorized audio recording, video recording, or photography is not permitted within the practice.
Clinical or practice-related photography or recording may occur only with appropriate authorization.
Submission of an insurance claim does not guarantee payment. Patients remain responsible for amounts assigned to them by their insurance plan and for services that are not covered.
We are committed to creating a welcoming, safe, professional, and respectful environment for our patients, visitors, and staff. We ask that all interactions be courteous. Abusive, threatening, discriminatory, harassing, or disruptive behavior toward our team or other patients will not be tolerated and will result in immediate dismissal from the practice, consistent with applicable law.
