Melasma

Melasma is a complex pigmentary disorder that requires a precise, long-term management approach. At Capital Skin and Laser in Columbia, South Carolina, we treat melasma with careful attention to underlying triggers, pigment depth, and skin type. Our approach prioritizes stability and gradual improvement, recognizing that successful treatment depends on consistency, sun protection, and thoughtfully selected therapies tailored to each patient.

What is Melasma?

Melasma is a common form of hyperpigmentation that appears as blotchy patches of light to dark brown discoloration on the face.  While medically harmless, melasma can be cosmetically distressing and often impacts self-esteem and confidence.

Melasma most often affects women between the ages of 20 and 40, particularly those with naturally tan or darker skin tones.  Many patients report a family history of melasma and a connection to hormonal changes such as pregnancy, oral birth control, hormonal IUDs or implants, or hormone replacement therapy.  In some cases, medications or personal care products may trigger pigment production.

How is Melasma Diagnosed and Treated?

Melasma is diagnosed with a careful skin examination. Occasionally, a special device called a dermatoscope is used to determine the depth of pigment in your skin.

Sun protection is the foundation of successful melasma management. Long-term, year-round protection, including daily broad-spectrum sunscreen, wide-brimmed hats, and sun-safe habits, is essential.

With consistent care, melasma can be significantly improved using carefully selected topical treatments such as retinoids, hydroquinone, azelaic acid, kojic acid, and other pigment-correcting agents. More advanced treatments, including oral medications, chemical peels, and laser treatments, must be considered with expert guidance, as some options can worsen melasma or lead to only temporary improvement.

“Dr. Spring and her staff were upbeat, attentive, and caring. They saw my anxiety prior to my procedure and took an extra moment to talk through it with me, which made me feel like they care about me as a whole person. I’ve never felt anxious prior to treatment before so this extra step really hit home.”
